How to be ready for your Medicare appointment.

WHAT TO BRING

Your Medicare appointment checklist.

Bring or have access to the following information when you meet with your advisor:

Current medications

Bring a complete list of your medications, including dosage and preferred pharmacy.

Doctors and providers

Bring a list of your doctors, specialists, clinics, and other providers.

Medicare card

Bring your current Medicare card and any existing plan information.

Medicare account

Have your Medicare.gov username and password available when possible.

5

Social Security account

Have your SSA.gov username and password available, especially if you are signing up for Part A and Part B for the first time.

Set up your online access before the appointment.

If you do not yet have these accounts, visit the official Social Security and Medicare websites to create them.

Let us know before your appointment.

If you are unable to obtain your login information, please tell us ahead of time so we can allow additional appointment time.
